Acute rheumatic fever (ARF) in people of all ages and rheumatic heart disease (RHD) in people aged less than 35 years became notifiable in NSW on the 2 October 2015.
NSW Health has established a register for people with ARF and RHD to improve the long-term clinical care and follow-up of these patients. The register is a voluntary register and consent should be sought using the appropriate consent form and other resources below to ensure patients are making an informed decision.

Once a patient has provided consent for the register a registration form should also be completed.
