HealthOne Auburn

The HealthOne Auburn hub has been developed as part of the new Auburn Community Health Centre on the Auburn Hospital site in the Sydney West Area Health Service (SWAHS). HealthOne Auburn opened in early December 2010, and operates under both the virtual, and the hub and spoke models.

The Auburn location and community

The Auburn Local Government Area (LGA) includes the suburbs of Auburn, Berala, Homebush Bay, Lidcombe, Newington, Regent’s Park, and Silverwater. The population of the LGA is among the fastest-growing in NSW, with an estimated population in 2008 of 73,495. In 2008, a large portion, 27.3%, of the Auburn population was under the age of 24, while only 8.5% comprised people over the age of 65.  A very large proportion: (59.1%) of the population of Auburn was born overseas. 

The Auburn LGA has the second lowest socio-economic status in Sydney. This has direct implications for the health of the local population. The Auburn LGA has a very high settlement rate for refugees as one of the top ten refugee settlement destinations in Australia, with a high proportion of permanent humanitarian arrivals to NSW.  For many within the community, access to health care from culturally sensitive health providers in their primary language is limited.

80 General Practitioners currently operate in 27 practices in the Auburn LGA.  Many work part-time, bringing the full time equivalent number of General Practitioners to 45. GP Services in Auburn are stretched, with close to 50% more patients accessing services than elsewhere in NSW. GP services and supply are recorded at the rate of 6.7 consultations per person. This rate of service is the third highest in Australia.

The service and target population groups

HealthOne Auburn brings together General Practitioners, Community Health staff and other service providers in the care of identified clients.  The service facilitates communication between service providers, supports care planning and coordinates client access to a wide range of health and human services. Two General Practice Liaison Nurses employed by Community Health – one for Chronic, Aged & Complex Care and one for Child & Family – will be the linchpins of this model, identifying clients needing care coordination and  linking General Practitioners, Community Health and other service providers.  HealthOne Auburn aims to meet the primary health care requirements of:

  • Clients with complex health needs, chronic illnesses or who are frail, aged and live at home
  • Refugee and Humanitarian immigrants in both the Complex Aged and Chronic Care and Child and Family population groups
  • Families with children where there are vulnerabilities / risk factors and a need for additional support
