HealthOne Corowa
HealthOne Corowa is currently under development, with completion anticipated in mid-2011. HealthOne Corowa will comprise conjoined general practice and community health facilities on the campus of Corowa Hospital.
The existing Corowa Medical Centre on the hospital campus is being extended by the practice principals, with the new facility being built by the Greater Southern Area Health Service to fit together with the existing facility so that the two separately-owned components will appear seamless to clients and staff.
The new HealthOne building is close to the main hospital building, providing the GPs (who are also GP Visiting Medical Officers) and community health staff ready access to patients and other health service staff.
The Corowa location and community

Corowa is a thriving rural community located on the Murray River to the west of Albury and Wodonga.

The catchment of the Corowa Hospital and health service encompasses approximately 20,000 people living in Corowa and outlying districts.

28.5% of the population is below the age of 25, in comparison with people over the age of 65 who make up 22.5% of the population.

The area offers a diversity of employment, combining rural agricultural and related industries (food storage and processing) with tourism, outdoor leisure, freighted transport, manufacturing, retail and trade.

The area is ideal for retirement and a popular recreation spot for those living in regional centres.
The service and target population groups
General practice, community health and non-government allied health services will be integrated together to provide primary health care across the spectrum of disciplines, with a particular focus on:

Mothers and babies

People with mental illness

People with drug and alcohol addiction
New models of care have been developed for HealthOne Corowa, and the following activities have either been planned and/or already commenced:

Weekly continuum of care meetings involving general practice nurse and practitioners, community and acute inpatient care staff, allied health practitioners, non-government service providers, aged care service providers and cross-border health service representatives;

Discharge Planning Project;

Avoidable Hospital Admissions Management Program, including service providers involved in Hospital in the Home, acute care and chronic disease management;

Pulmonary and cardiac rehabilitation; and

Clinics and health promotion/prevention activities, including:
