HealthOne Elderslie
The key aim of HealthOne Elderslie is to provide quality community based chronic disease management with the intention of reducing hospital demand. A community health nurse has been located at the practice since August 2007. HealthOne Elderslie provides a comprehensive range of services including primary prevention, early recognition and early intervention, secondary prevention and the structured management of established chronic illness. Other health services co-located at the practice include pharmacy, dental, psychology, audiology, occupational therapy and pathology.
While the community nurse continues to provide traditional services, the role also includes facilitating multidisciplinary activity involving chronic disease specialists such as diabetes educators, cardiovascular nurses, mental health nurses and palliative care. Closer liaison with acute care services, including hospital discharge planners, has also been established. Co-ordination of health promotion events at the practice is another aspect of the role.
The next phase is to offer ambulatory care where possible, involving liaison with the local ambulatory care service as necessary.
