HealthOne Rouse Hill

HealthOne Rouse Hill will be located in the new Rouse Hill Town Centre and will be a hub and spoke model
 
The hub will be a central operational base for delivery of prioritised clinical services, care coordination and integrated primary health care services. The spokes represent integration of primary health care services through new ways of working and linkages with GP practices through technology, care navigation arrangements, partnerships and GP liaison.
 
The multidisciplinary team will be comprised of General Practitioners, Community Nurses (Chronic and Complex and Child and Family), sessional Allied Health, (therapy & counselling) and mental health professionals

Location and demographics

  • The Rouse Hill region is an area that incorporates large sections of Blacktown and Baulkham Hills Local Government Areas (LGA).  These two LGAs have been consistently in the top 3 fastest growing LGAs in NSW for the last 10 years.  Both LGAs will continue to grow rapidly.
  • In 2001 Hawkesbury Hills Division of General Practice (HHDGP) had a population of 228,000 residents.  By 2006 this had grown to 261,755.  The population increased by 33,755 residents over a 5 year period or 6,751 residents per annum.  HHDGP has a relatively young population, with an average age of 32 compared to the national average of 36, however there is a significant over 70 and over 85 population, with Baulkham Hills Shire Council having the fastest growing over 85 population in NSW.

  • People with complex and chronic health issues e.g. respiratory, heart failure, diabetes
  • Vulnerable and at risk young families
  • People who frequently present to hospital Emergency Departments
  • People without a GP
  • Aboriginal Community in nearby Riverstone
