Information for GPs

Healthy at Home supports the care of older people (65 years and older or 45 years and older for Aboriginal and Torres Strait Islander people) whose medical condition is starting to deterioriate. It provides care in the person's home as an alternative to possible hospital admission.

Patient eligibility:
 
  • Aged 65+ years or 45+ years for ATSI.
  • Lives in the following local government areas:

St George

Hornsby/Ku-ring-gai

Queanbeyan

Greater Newcastle area

  • Had recent deterioration that may lead to a hospital hospital admission.

Accepted referral can result in:

  • assessment within 48 hours
  • prompt access to diagnostics and clinical management
  • up to 6 weeks of community case management
  • establishment of long term sustainable care.

When to refer:

  • increasing number of surgery presentations
  • illness status - sub acute
  • deteriorating function/mobility/cognitio
  • falls
