Co-Case Management
ADAHPT is a statewide tertiary outreach team that offers a range of services for people living with HIV/AIDS and complex needs, including co-case management. Co-case management involves supporting the local case manager to coordinate care, treatment and support services owing to statewide responsibilities that are relevant to the changing needs of the individual.
ADAHPT workers are unable to take on the role of primary case manager as we are a small team that covers the whole of NSW. Where a client without an identified case manager is referred to the service, ADAHPT will work with the referring agency and the client to locate an appropriate service provider who is able and willing to act as the person's case manager. To avoid confusion and maintain a central point of reference, it is still preferable to distinguish between the case manager as the primary worker with a secondary supportive role being undertaken by the co-case manager. Successful co-case management relies upon clear negotiation and understanding of each workers respective roles and responsibilities, shared goals for client care and ongoing communication.
The service is able to provide co-case management by providing to case managers, carers/friends/relatives and other services, the following assistance:
- Advice and information
- Assistance with development and implementation of a treatment, care and support plan
- Case specific education and training
- Assistance with definitive diagnosis for complex cases
- General education and training
- Provision of resource materials
- Peer support for people working in the area
- Neuropsychological Assessment
- Brokered Care
