Brokered Care

ADAHPT's brokered care is the funding of a private agency care worker to complete tasks specified within a client's case management plan, when other services are not available to assist with these tasks. The brokered care worker typically helps with behavioural modelling and/or supports the client to re-learn skills to perform daily tasks. This may include one or more of the following: assistance with shopping, cleaning, social activities, transport to appointments, cooking, household prompting, respite for relatives, and personal care.

Brokered care can be considered by ADAHPT only if the Community Options Program or HACC and/or other support arrangements have been proactively and fully explored in the first instance, and the case management plan cannot be implemented without the funding of a brokered care support worker to fulfil these nominated tasks.
