Adahps Service Delivery Model

Service Delivery Model

The ADAHPS model comprises a comprehensive state-wide network of services established to provide specialist consultation, assessment, diagnosis and management for people with complex needs usually associated with a diagnosis of AIDS Dementia Complex (ADC), or cognitive impairment and/or a HIV related psychiatric condition.
The service is auspiced by South Eastern Sydney/Illawarra Area Health Service (SESIAHS) and Sydney South West Area Health Service (SSWAHS).

Core services within the network include:
  • The Bridge', an 11 bed residential facility providing 24 hour supervised high level nursing care. One bed is for respite. The facility is in Glebe and is administered by the Royal Prince Alfred Hospital (RPAH) and SSWAHS.
  • Sacred Heart and St Vincent's Hospital (SVH) which provide palliative care, rehabilitation, stabilisation and respite beds.
  • St Vincent's Hospital and RPAH Mental Health Services, provide mental health inpatient and outpatient care.
  • High supervision bed at SVH, in special circumstances and on a case by case basis.
  • ADAHPT, a state-wide outreach team providing neuropsychology, assessment, care planning, co-case management, supported accommodation, brokered care,Telehealth, and targeted education services. The service also has other supported accommodation arrangements with a range of housing associations.

All clients to Adahps are referred through ADAHPT, which is also the point of referral for all clients to the service.

Several HIV/AIDS funded clinical services provide professional and organisational support and services to ADAHPS and clients. These services include:

  • HIV Neurologist and Neuro-Psychologist, St Vincent 's Hospital,
  • HIV Liaison Psychiatry Service, Royal Prince Alfred Hospital,
  • HIV/AIDS Service, Immunology Department, RPAH,
  • HIV/AIDS Co-ordinators at SSWAHS and SESIAHS provide inter-sectoral coordination and planning support.

The Adahps Co-ordinator's role includes service coordination, administration, budget management, planning, service development, clinical leadership and is the team leader of ADAHPT.
