Specialist Gynaecological Oncology Units in NSW

Gynaecological cancer accounted for 1360 new cases of cancer in women in NSW in 2005 and are the fourth most common cancer in women (Tracey et al. 2007)

The Australia Cancer Network / National Breast Cancer Centre Ovarian Cancer Guidelines of 2004 quote, "There is a growing body of evidence to suggest that outcomes for a woman with ovarian cancer are improved if she is referred to be managed under the care of a Multidisciplinary Care team (MDT)". A recent systematic review undertaken by Vernooij and colleagues in 2007 concluded that the outcomes of ovarian cancer treatment are better when provided by gynaecologic oncologists in tertiary units.

Of great concern to the gynaecological oncology community however is a recent study of patterns of care reported by Grossi and colleagues in Victoria which showed that primary surgery was performed by gynaecological oncologists in only 47.2% of cases. A similar picture exists within New South Wales.

Palliative care issues for women with gynaecological cancer are often complex and require specialist intervention that may not be readily available. Recent research by Herrinton and colleagues in 2007 reported that end of life care is often inadequate. Myriad symptoms are experienced including bowel obstruction, oedema, malnutrition, ascites, ureteric obstruction, fungating wounds and pelvic pain syndromes. Such a high symptom burden may explain why 31% of women with advanced gynaecological cancer die in hospital while awaiting transfer to hospice or discharge home (Dalrymple et al. 2002).
