Resources For General Practitioners

Spinal Outreach Service Health Questionnaire (SOS-HQ) | Journal Publications

People with spinal cord injury (SCI) can have specialised needs and complex issues affecting multiple body systems. A range of resources are available to assist the general practitioner in managing issues related to SCI.

Spinal Outreach Service Health Questionnaire (SOS-HQ)

This tool helps identify new problems, alert to red flags and prompt preventive care activities. It can be used to develop a General Practice Management Plan (GPMP) and plan Team Care Arrangements (TCA). Covering the SOS-HQ in sections can facilitate a 12-month cycle of preventive health care, allowing systematic monitoring and increasing intensity/coordination of professional input for those identified with greatest need.

The SOS-HQ is designed to be completed in part by the patient, the practice nurse and the general practitioner for maximum efficiency. It uses lay language and is typical of what spinal specialists would ask in a patient consultation. It is divided into 12 sections, which can be covered in 4 separate consultations. You can also download it in its entirety.

Sections of the SOS-HQ

Suggested Format of completing the SOS-HQ in a year to complete a cycle of care (each questionnaire includes a cover page, relevant sections, and a management plan page)

The SSCIS has also compiled a range of clinical resources.
