Evaluation of a manual wheelchair
- Product: Make, model and code
- Size: see sample form for specifications of seating and manual wheelchair
- Age of the wheelchair
- Condition: identify rust, wear and tear issues on the frame and component parts when assessing for replacements
- Features/specifications: see Module 9 for more details.
- Drive type: self-propelled/transit
- Frame type: folding, rigid, tilt in space, recline or bariatric. Frame material used: Steel, aluminium, titanium, alloy
- Adjustability: axle mounting plate, back cane angle and position, footplate hangers such as elevating or stump support
- Rear wheels: diameter, width, profiles and tyres types, rear wheel cambers, wheel release type, push rim and spoke type
- Wheel lock: push or pull to lock, high or low mount, scissors, toggle
- Casters: diameter, width, profiles and tyres types
- Armrests type: removable, flip up, desk, adjustable, single/dual mount
- Footplates: single or dual, rigid platform, tubular or flip up, angle adjustment
- Others: anti-tippers, push handles, transport options, spoke guards, clothing guards, suspension
