NSW Aboriginal Chronic Conditions Area Health Service Standards
The NSW Aboriginal Chronic Conditions Area Health Service Standards have been developed by NSW Health in partnership with the Aboriginal Health and Medical Research Council of NSW to facilitate improved Aboriginal health outcomes by optimising the accessibility and appropriateness of health services for the prevention and management of chronic conditions in Aboriginal people. The Standards augment the NSW Chronic Care Program Clinical Service Frameworks in heart failure, respiratory disease and cancern and take a broader approach to include cardiovascular disease, diabetes, kidney disease, chronic respiratory disease and cancer in Aboriginal people.
