NSW Chronic Care ProgramClinical Service FrameworksThe Clinical Service Frameworks were developed by the NSW Chronic Care Program to improve the care of people with chronic and complex conditions. They provide practical, evidence-based and flexible approaches to the management of heart failure, respiratory disease and cancer. The frameworks:
NSW Clinical Service Framework for Heart FailureThe NSW Clinical Service Framework for Heart Failure Executive Summary, Volume 1 and Volume 2 outline best practice approaches for the management of patients with heart failure. Standards cover issues such as prevention, identification and management of factors that precipitate and exacerbate congestive heart failure (CHF), assessment and diagnosis, essential components of multidisciplinary care, best practice pharmacological and non-pharmacological management, self-management, rehabilitation and palliative care. NSW Clinical Service Framework for Chronic Respiratory DiseaseThe NSW Clinical Service Framework for Chronic Respiratory Disease consists of the Executive Summary, Volume 1, Volume 2 and Quick Guides. NSW Clinical Service Framework for Chronic Respiratory Disease Volume 1 outlines best practice standards, targets and milestones for COPD and asthma for NSW Area Health Services. NSW Clinical Service Framework for Chronic Respiratory Disease Volume 2 is a practice guide for the optimal treatment of chronic respiratory disease and provides the latest best practice standards and models of care for the management and treatment of people with COPD and asthma. It also covers issues such as the use of spirometry, smoking cessation, pulmonary rehabilitation, influenza vaccinations, home oxygen therapy and asthma action plans. Quick Guides summarise and highlight the most appropriate management approaches for COPD and asthma in hospital and community-based settings. NSW Clinical Service Framework for Optimising Cancer Care in NSWThe NSW Clinical Service Framework for Optimising Cancer Care in NSW and its Executive Summary draw on the evidence-based literature for optimal cancer care and involved widespread consultation and endorsement by senior clinicians and consumers across NSW, including the NSW Area Cancer Services Directors Forum. The framework builds on the NSW Health Optimising Cancer Management Initiative, including the Cancer Care Model for NSW introduced in 1999.
