Incident reporting
In accordance with legislation licensed private health facilities are required to report injuries, transfers and deaths using the prescribed PR17: Incident report form. Serious incidents are to be reported within 24 hours or the next working day, all other incidents within 30 days.
It is recommended that facilities have regard to NSW Health Policy Directive PD2007_061: Incident Management when reporting incidents.
It is not a requirement under the Regulations to report infection control incidents, but we recommend that facilities report infection control breakdowns that involve potential patient exposure to an infection.
