The Ongoing Needs Identification (ONI) screening tool is designed to prompt timely and appropriate service delivery, referral and/or further assessment based on the issues and needs that are identified for each person. This tool is not just an eligibility screen. It is designed to be used in a range of primary health care programs and settings. The aim of screening is to differentiate between people who:

  • Have no problems and need no services.
  • Have minor problems (i.e. low need), need some services (eg, meals, home maintenance), but do not need a full comprehensive assessment.
  • Have medium to high needs and require a full assessment.
