What is Healthy at Home?
Healthy at Home is a program designed to support people aged 65 years and older in their home (or 45 years and older if Aboriginal and Torres Strait Islander).
Healthy at Home enables you to have access to health care and community services when your health is declining.
This service is provided before you get to the stage of requiring a hospital presentation.

What happens after I am referred to the Healthy at Home team?

The Healthy at Home team will call you by phone before visiting you at home.

Please restrain pets and do not smoke when the team comes to visit you.
What services are involved?
Depending upon your circumstances, the Healthy at Home team can provide:

Clinical services such as medical, nursing, physiotherapy and occupational therapy to manage your health needs at home

Fast access to blood tests and x-rays, and/or
-
Community Services (ComPacks) such as housework, transport, heating of prepared meals and personal care (depending on your specific needs).
How long will Healthy at Home be involved in my care?
The Healthy at Home team is involved for up to six weeks. During that time referrals will be made to other service providers as required, for example home care, meals on wheels etc.
What information is shared about me?
Staff will use or disclose your information only for purposes directly related to your ongoing care.
The Healthy at Home team member will inform you of any referrals made to a service, their contact details and role.
Your GP will be contacted about your progress and involved in any decisions about your treatment / management, unless you request otherwise.
