The application process is a three stage process:

Stage One: Online Eligibility and Commercial Opportunity
Stage Two: Preliminary Application Form
Stage Three: Full Application Form (invitation only)

Applicants will self-determine whether they proceed from stage 1 to stage 2, and full applications (stage 3) will be by invitation only. The purpose of these forms is to enable applicants to provide information in a consistent format.

All applicants are required to complete the self-assessment survey online prior to submitting a preliminary application form.

Preliminary applications close 5.00pm Friday, 12 February 2016.

OHMR require all twelve (12) hard copies of the preliminary application to be formally lodged. (i.e. in person, signed courier or registered post).
