NSW Oral Health ​Fee for Service Scheme

The Oral Health Fee for Service Scheme allows public dental service patients to receive dental care through a voucher system.

Dentists, oral health and dental therapists, dental hygienists, dental prosthetists are eligible to participate in the Scheme.​ Each Local Health District is responsible for clinician registration, voucher distribution and voucher payment.

Private providers enquiring about the scheme, including claims questions, should contact the c​oordinator for the LHD who issued the voucher. Contact numbers can be found on the provider brochure, or you can contact through the oral health call centre for the LHD. Coordinator numbers are for provider enquiries only, and patients must use the oral health call centre numbers.

Referr​al forms

Treatment Referral Form
This form is to be used by private providers who are registered with the OHFFSS and are referring patients back to a public dental clinic for further oral health care.

Emergency Department Referral Form
This form is used by hospital emergency departments to refer patients for oral health care.

Oral Health Specialist Referral Protocols
Outlines the requirements and eligibility for referring patient for specialist oral health services in the NSW Public Oral Health Service.

Oral Health Specialist Referral Form
This form is to be used by private and public oral health and medical practitioner(s) who are wishing to refer their patient to be assessed or receive oral health specialist care as per Oral Health Specialist Referral Protocols PD 2001_071.

Oral Health Treatment Referral Form
This is to be used by private and public oral health and medical practitioners who are wishing to refer their patient to be assessed or receive oral health care through generalist services​. This should not be used in association with the Oral Health Fee For Service Scheme (use Oral Health Fee For Service Scheme - Treatment Referral Form).
