Summary
This document is to be used to guide the development of standardised medical discharge referral reports. It contains a minimum set of items that will enable the recipients (GPs, specialists, other facilities) to know what to expect, and how to interpret it, irrespective of local differences in the formatting and presentation of a referral report. The standard can be applied to electronic or paper based discharge referral reports. It supports the state baseline build for the Electronic Medical Record (eMR) and is consistent with the minimum requirements for discharge referral specified in the Policy for Discharge Planning: Responsive Standards PD2006_054
