Summary
The NSW Aboriginal Chronic Conditions Area Health Service Standards establishes standards of practice for Area Health Services for the prevention and management of chronic conditions in Aboriginal people. It is expected that Areas will develop plans to meet the standards through local partnerships, in particular, Local Area Aboriginal Health Partnerships, to ensure the input of the Aboriginal community and Aboriginal Community Controlled Health Sectors.
