Summary
This Policy Directive describes procedures for review of perinatal deaths at hospital level and revises the process of classifying perinatal deaths and statewide reporting to be implemented from 1 January 2006. PD2005_641 has been rescinded and this document is issued to incorporate amendments on Pages 3 & 4, Appendix 1.
