Summary
Outlines the published evidence concerning rehabilitation for chronic diseases such as cardiovascular disease, respiratory disease, diabetes, arthritis, cancer and others. This policy aims to support the goals of rehabilitation through implementation of self-management support strategies, behaviour change interventions to deter or retard chronic disease, and optimal health for people with chronic disease to enable them to live productive lives in their community. Recommendations for Area Health Services include the review of current services in order to provide safe, accessible and effective rehabilitation for these patient groups in local communities. The document is accompanied by Vol 2 "Implementing Rehabilitation for Chronic Disease" GL2006_022, which provides information regarding resources that may be used in the delivery of rehabilitation for chronic disease.
