Access to Psychiatric Patient Medical Records

How to access

To assist private individuals in applying for access to psychiatric patient medical records, the department requires certain information in order to assess the application. The applications usually fall into one of the following four categories.

To view the assessment criteria, click on the sentence that matches your family research situation:

  1. Access medical records of a person from whom you are directly descended (eg. parent, grandparent, great grandparent)

  2. Access medical records of a person from whom you are not directly descended but are related to and who has descendants
    (eg aunt, uncle, great aunt, great uncle, cousins)

  3. Access medical records of a person from whom you are not directly descended but are related to and who does not have descendants
    (eg aunt, uncle, great aunt, great uncle, cousins)

  4. People who are adopted and are seeking access to the medical records of their birth parent/s
