Quitline Referral forms

Summary

Recently published NSW Health clinical frameworks for Chronic Respiratory Disease and Heart Failure (NW Health 2003) outline targets for provision of referral to smoking cessation services of all patients who smoke. The Quitline fax referral forms will be printed in pads of 50 and enable health services to provide informed consent and arrange for the Quitline to provide assistance to patients/clients who would like help to quit smoking. Referred patients will be contacted by the Quitline at a day and time of their own choice.
