Summary

Phase Two of the NSW Chronic Care Program (2003-2006) provides opportunities to build on initiatives and lessons learnt during Phase One. Key components of Phase Two are the establishment of new governance arrangements (including the overarching Chronic Care Implementation Committee and new Clinical Expert Reference Groups) and the continuation of the Chronic Care Program Managers Forum.
