My First Health Record - NSW Child Personal Health Record
SummaryThe May 2008 version of the NSW Child Personal Health Record (PHR) incorporates a number of changes which were identified through a process of clinician and parent review. The printed version, distributed at birth to each baby born in NSW, includes the Parent Evaluation of Developmental Status (PEDS) in each health check from 6 months. The PEDS does not appear in the electronic version.
