Summary

Tracking Tragedy 2008 is the fourth report of the NSW Mental Health Sentinel Events Review Committee. The Committee was formed in 2002 to help to identify systemic problems and advise on opportunities for improving the safety and quality of service delivery for consumers of mental health services and the wider community. The Committee's fourth report focuses on suspected suicide deaths reported by NSW Mental Health Services between 2003 and 2006, where the person was an inpatient of a NSW public mental health unit, homicides that occurred during 2005 and 2006 as well as serious non fatal injuries caused by patients of mental health services in the years 2004 to 2006.
