The NSW Government's Final Response to Tracking Tragedy 2008 reviews and responds to the recommendations made within the fourth report of the NSW Mental Health Sentinel Events Review Committee. The Committee was formed in 2002 to help identify systemic problems and advise on opportunities for improving the safety and quality of service delivery for consumers of mental health services and the wider community.
