My Health Record

TThere is a number of publications associated with My Health Record and hard copies are available from community health centres or health service providers.

My Health Record is an A5 size booklet which was designed as a paper-based folder with inserts that are divided by sections and have been colour coded. There are:

  • Red tags for you and/or your carers to complete, for sections:
    - Section 1 : Personal details
    - Section 2 : Emergency contacts
    - Section 3 : Key health and other contacts
    - Section 5 : Monitoring your health problems
    - Section 9 : Advance Care Directive

    Refills for these sections are available through health service providers or can be ordered from the NSW Better Health Centre by sending the fax order form.

  • Gold tags for you and your health services providers to complete together:
    - Section 4 : Appointments
    - Section 6 : Health problems
    - Section 7 : Allergies, side effects and immunisation / vaccinations
    - Section 8 : Medications

A number of plastic sleeves are available at the back of the booklet. These plastic sleeves are for storing copies of records such as prescription, emergency details, medical history and diagnoses, test results, treatments, assessments, care plans and business cards. These also can be used to store copies of Medical Director reports from your family doctor.

Information and guidelines on how to use My Health Record are in every section.

A Patient feedback form has been inserted into the front sleeve of the record. You are advised to return the form to NSW Health free of cost (no stamp required). The purpose of the form is to evaluate the use and acceptance from consumers. This will be useful in improving My Health Record in future.
