My Health Record is an A5 size booklet which was designed as a paper-based folder with inserts that are divided by sections and have been colour coded. There are:
A number of plastic sleeves are available at the back of the booklet. These plastic sleeves are for storing copies of records such as prescription, emergency details, medical history and diagnoses, test results, treatments, assessments, care plans and business cards. These also can be used to store copies of Medical Director reports from your family doctor. Information and guidelines on how to use My Health Record are in every section. A Patient feedback form has been inserted into the front sleeve of the record. You are advised to return the form to NSW Health free of cost (no stamp required). The purpose of the form is to evaluate the use and acceptance from consumers. This will be useful in improving My Health Record in future.
