Easy Guide to Clinical Incident Management - Step 2. Notification

  1. All incidents must be notified immediately to your line manager.
  2. You must also create a Clinical Incident Form in the incident Information Management System (IIMS) as soon as practicable and preferably before the end of the working day. Contact your IIMS manager if you need assistance or see the list of local administrators in the IIMS intranet website.

In IIMS...

... complete the mandatory fields first. If you are called away from the computer, you can click the Save button and provide details to your manager later.

If you are a manager...

  1. Email a Reportable Incident Brief (RIB) for all SAC 1 incidents to the NSW Department of Health within 24 hours of the incident.
  2. Address the email to ESUMAIL@doh.health.nsw.gov.au under the CE's or nominated delegate's signature.
  3. If the issue is urgent, provide telephone advice to the Executive Service Unit at (02) 9391 9621 and to
    the Department's Media and Communication branch on (02) 9391 9121 that a RIB has been emailed.
RIBS for SAC 2, 3 and 4 incidents are only sent to the Department if there is a statewide risk to patient safety.
