Application for qualified privilege

To apply for qualified privilege, a representative of the committee must complete the Application for Qualified Privilege by a Quality Assurance Committee form and submit it to the Quality and Safety Branch at the NSW Department of Health, as indicated in the form.

The form is in Microsoft Word. Please save it to your local folder, complete it online, then print and sign it.
