HIV/AIDS Case Management and Referral Project 2010 -2011

Summary

With the benefits achieved through improved HIV treatments over the last decade, the nature of disease progression has changed HIV/AIDS from being an acute medical condition to a chronic one with at times complex clinical and behavioural care needs. In the current context approximately 70% of people with HIV report living well and only require routine monitoring of their HIV infection. Of the remaining 30%, there are many whose condition requires linkages and support between a range of services including, for example, medical and psychological support as well as support in accessing housing, financial planning and a range of other welfare/daily living services, often packaged in a way that crosses traditional regional boundaries and address multiple needs in addition to HIV infection.  These services are tailored to individual need to enhance the quality of life and continued well-being.
