Presentation for GPs: The Patient Journey Kit1 'the transfer of stable public clinic opioid dependent patients to GP prescribers that supports' the development of a combined care and business plan. The kit has been developed to maximise the appropriate utilisation of existing care and referral pathways and optimise the quality of care delivered to patients and the financial renumeration obtained by those delivering care in the community, specifically GPs, but also pharmacists and allied health professionals.
