NSW Privacy Form for Researchers
Summary
This form is used by Researchers applying to Human Research Ethics Committees (HRECs). A researcher is required to complete this form so that the HREC can assess the research application under the Health Records and Information Privacy Act and meet its obligations under the Act when it reports to the NSW Privacy Commissioner.
