Summary

This form is used to report summary information about gastrointestinal and foodborne outbreak investigations to OzFoodNet. Definitions of outbreaks that are included in this surveillance scheme are included in the guide at the end of the form. Completed forms should be faxed or emailed to the OzFoodNet epidemiologist in your State, or the OzFoodNet Coordinating Epidemiologist.
