Application for Authority to Prescribe Dexamphetamine or Methylphenidate for a Child or Adolescent

Summary

Under Section 28 of the Poisons and Therapeutic Goods Act 1966, the authority of the NSW Ministry of Health is required to prescribe for, or supply to, any person any preparation of dexamphetamine or methylphenidate.


An Application for Authority to Prescribe a Dexamphetamine or Methylphenidate - Form 1A - is to be used for the purposes of applying for an authority from the Ministry to prescribe for a child or adolescent with Attention Deficit Hyperactivity Disorder.


The prescribing of CNS stimulant medication for the treatment of Attention Deficit Hyperactivity Disorder is to be in accordance with the latest version of TG 181, Criteria for the Diagnosis and Management of Attention Deficit Hyperactivity Disorder in Children and Adolescents


Form 1A may be completed electronically using Adobe Reader. (The latest version of Adobe Reader can be downloaded for free from Adobe).  Save the form to a local hard drive before typing in details.
