Notification of prescribing of dexamphetamine and methylphenidate for ADHD

Summary

This notification form is to be completed by generally authorised prescribers who prescribe dexamphetamine or methylphenidate for persons with Attention Deficit Hyperactivity Disorder (ADHD).  Prescribing must be in accordance with Criteria for the Diagnosis and Management of Attention Deficit Hyperactivity Disorder in Children and Adolescents (TG 181) and Criteria for the Diagnosis and Management of Attention Deficit Hyperactivity Disorder in Adults (TG 190).  All prescriptions that are issued using a general authority number are to be notified to Pharmaceutical Services on a monthly basis.


The form may be completed electronically using Adobe Reader. (The latest version of Adobe Reader can be downloaded for free from Adobe).  Save the form to a local hard drive before typing in details.


Completed forms are to be faxed to Pharmaceutical Services on (02) 98595175 or posted to Pharmaceutical Services, NSW Ministry of Health, PO Box 103, Gladesville NSW 1675.
