Central Coast Local Health Districts' ComCare Electronic Medical Record (ComCare eMR) system is enabling real time, point of care electronic documentation and reporting for those with chronic wounds.

The ComCare eMR system used in Community Complex and Chronic Care services provides a number of benefits for clinicians and patients across the district including:

  • ease of access and sharing – point of care documentation and digital wound photography can be viewed remotely in real time by wound experts to enable effective and efficient wound care escalation and care planning
  • streamlined data collection and reporting on key metrics  purpose built dashboards enable tracking of length of stays, wound types and numbers of wounds
  • dedicated dashboard  shows key dates for client assessments, electronic wound pathways and more.

A key benefit of ComCare eMR is its facilitation of the wound consumable prescription system. It enables nurses to order wound consumable products for individual clients at the point of care. If an antimicrobial or high cost item is ordered, the system flags the order and client for remote review by a wound care expert to ensure the order is suitable. This allows an audit and compliance check on evidence based practice prior to the item being individually packed and delivered by care staff.

ComCare eMR has enabled a proficient wound care service that enables real time point of care electronic documentation and reporting in the district.
