​Clinical information to provide

Once a prescriber has agreed to take over care for your client, the following information should be provided to enable safe transfer of care:

A summary of treatment provided, including:

  • medication(s) prescribed and/or dispensed
  • last dose details
  • number of take-away doses or suitability for take-away doses
  • dosing point contact, if different

Client goals of treatment, including progress to date.

Significant co-morbidities (physical health, mental health).

Other medications prescribed (by OAT prescriber or GP).

Current concerns and risks (inc. other substance use, child safety, homelessness, high risk behaviours etc).

Provide contact details in case follow up information is required. Ensure client's contact number is up to date.

Patient changing prescribers

Ensure there is enough supply left on the current prescription to allow client to make an appointment with new prescriber. Complete an exit form for clients* that are transferring from your care.

Clients must be exited from treatment with one prescriber so that another prescriber can be issued with an authority to prescribe or supply to them

Patient changing dosing points only

Notify the NSW Ministry of Health of any permanent change to a NSW OTP client's dosing point using  Notification of Permanent Change in Dosing Point Form.
