The Intersectoral Homelessness Health Strategy is a multi-agency initiative to improve the health outcomes of people experiencing homelessness in inner Sydney. This population group experiences significant barriers to healthcare and disproportionately high morbidity and mortality rates, with 22% of people sleeping rough in NSW located within South Eastern Sydney Local Health District and Sydney Local Health District.
The Strategy focuses on improving access to services, care coordination and strengthening joint responses across health, housing and social services. It shows how organisations can work together to address health inequities and deliver integrated, person-centred responses across prevention, primary care and public health.
Kellett Street Connect demonstrates this model in practice, delivering a trauma-informed, culturally safe, women-focused service that blends primary care, housing, legal and psychosocial supports. Early results show strong engagement among women and gender-diverse people who experience homelessness and face significant barriers to care.
