​​​​The Virtually Enhanced Indigenous Chronic Disease program adapts existing models of virtual care to better meet the needs of Aboriginal patients who often disengage from mainstream services.

The program aims to reduce emergency department (ED) visits and hospital admissions by supporting patients in the community. Patients are proactively identified following presentation to the ED, including those who leave before completing treatment. Patients can then receive care safely at home through remote monitoring, virtual consultations, and coordinated multidisciplinary care that is culturally responsive.

​The service combines clinical care with Aboriginal health and trauma-informed practice. Care is voluntary, patient-led and delivered in a way that prioritises trust, safety and choice. This transforms care from episodic and reactive to continuous and person-centred, improving patient experience, engagement and outcomes.
