The South Western Sydney Local Health District Aboriginal Chronic Care Program (ACCP), has operated since 2002 from Budyari Aboriginal Community Health Centre. The ACCP includes culturally safe community health centres, strong partnerships with Tharawal Aboriginal Medical Service and Gandangara Health Service, and tools like the risk stratification tool that integrates cultural, psychosocial, and clinical factors to tailor care.  

The ACCP coordinates care that includes home visiting and access to additional services that support people to manage their chronic condition. In 2023–24, the ACCP supported more than 6,200 consumers with care coordination and more than 5,300 with additional services. Patient feedback showed high satisfaction. More than 95% of patients felt involved in their care and treated with respect. The program has reduced hospital stays by enabling timely community-based care.  

Since 2019, the ACCP's client base has grown by 56.4%, with a shift toward low-intensity clients.​
