The Nurse-led Geriatric Oncology Model of Care (NL- GOMOC) was co-designed with consumers and implemented over 12 months.
The model focused on identifying and managing age-related health issues often missed in standard oncology care. Geriatric screening and assessments were done using validated tools within the electronic medical record and patient management was organised through multidisciplinary team meetings that involved oncologists and allied health professionals.
One hundred patients (median age 73 years) were assessed, with 88 receiving cancer treatment. Comparative analysis of 172 patients showed a 22% reduction in unplanned hospitalisations, a 50% decrease in toxicities related to chemotherapy treatment, and reduction in emergency department presentations. Patient feedback was positive with 98% expressing high satisfaction and 90% of staff believed it was a suitable way to provide care.
This model improved clinical outcomes and was successfully integrated into routine oncology practice.
