Chronic diseases are lasting conditions with persistent effects. They include some cancers, heart disease, diabetes, chronic respiratory diseases and mental health conditions. Chronic diseases impact on the physical, emotional and mental wellbeing of individuals, and may affect daily activities, relationships and employment.
