Aim: Renal supportive care (RSC) is a nurse-led alternate therapeutic strategy for patients with severe chronic kidney disease (CKD). It also focuses on managing the high physical and psychosocial symptom burden for patients and carers.
 
Method: It is a multidisciplinary coordinated service provided at inpatient, outpatient and home-based environment. Hospital and community staff education is also provided.
 
Results: RSC provides patient centred care with patient and carer satisfaction. RSC patients have reduced number and length of hospital admissions. Staff education is valued.
 
Conclusion: RSC provides value-based patient-centred care. Meanwhile providing financial savings making it a sustainable and effective service.
