Safe and Supported at Home (SASH) provides short-term support for people, under 65 years of age with functional impairments who are experiencing an exacerbation of a chronic or complex health condition and are at risk of hospitalisation. SASH packages are available for eligible people accessing Local Health District (LHD) and other community-based services. Support is generally provided for up to six weeks and may be repeated based on assessed need; however, there is a limit on the number of SASH episodes that can be accessed each year.
The SASH program is currently under review, and information about package duration and episode limits may be updated once the review is finalised.
