Safe and Supported at Home (SASH)

​​​​Safe and Supported at Home (SASH) provides short-term support for people, under 65 years of age with functional impairments who are experiencing an exacerbation of a chronic or complex health condition and are at risk of hospitalisation. SASH packages are available for eligible people accessing Local Health District (LHD) and other community-based services. Support is generally provided for up to six weeks and may be repeated based on assessed need; however, there is a limit on the number of SASH episodes that can be accessed each year.​

 

The SASH program is currently under review, and information about package duration and episode limits may be updated once the review is finalised.
