Attention Deficit Hyperactivity Disorder (ADHD) in a child, adolescent, or adult
The application for the treatment of ADHD in an adult has now merged into one form – a combined application form for ADHD for children, adolescents, adults, and high dose.
To apply for authorisation to prescribe or supply a psychostimulant (dexamfetamine, lisdexafetamine or methlyphenidate) for the management of ADHD in a child, adolescent, or adult patient, complete an
Application for authority to prescribe or supply a Schedule 8 drug - Psychostimulant for Attention Deficit Hyperactivity Disorder (ADHD).
The same application form may be used to prescribe a high dose of psychostimulant for the management of ADHD in a child, adolescent, or adult.
Psychostimulant for treatment of an indication other than ADHD
To apply for authorisation to prescribe a psychostimulant for the management of a diagnosis other than ADHD in a child, adolescent, or adult patient, complete an

Alprazolam and flunitrazepam
To apply for authorisation to prescribe or supply alprazolam or flunitrazepam, complete an

NSW Opioid Treatment Program (OTP)
Applications for authority to prescribe or supply methadone, buprenorphine or other opioid agonist therapy (OAT) treatment under the NSW OTP are to be made using one form (rather than a separate form for each drug). To apply for authorisation complete an
Application for authority to prescribe methadone, buprenorphine or other opioid agonist therapy (OAT) treatment under the NSW Opioid Treatment Program (updated form).
Please note that from 22 May 2023 the old OTP forms will no longer be accepted.
Prescribers wishing to apply for authority to prescribe or supply high dose methadone or buprenorphine please use
Application for authority to prescribe or supply methadone (>200mg/day) or buprenorphine (>32mg/day) under the NSW Opioid Treatment Program (updated form).
Interstate prescribers who wish to temporarily transfer a patient to the NSW OTP are required to complete an

To exit a person from the NSW OTP complete an
Exit from methadone, buprenorphine or other opioid agonist therapy (OAT) treatment under the NSW Opioid Treatment Program.
Certain restricted substances
To apply for authorisation to prescribe the certain restricted substances (acitretin, clomifene, cyclofenil, dinoprost, dinoprostone, etretinate, follitropin beta, hydroxychloroquine, isotretinoin for oral use, luteinising hormone, tretinoin for oral use, urofollitrophin (human follicle stimulating hormone)) complete an
Application for authority to prescribe certain restricted substances.
The
Authority under the Poisons and Therapeutic Goods Regulation 2008 enables prescribers at a public or private hospital to direct the administration of certain restricted substances to an inpatient commenced on the medication in the community.
Unregistered or (pharmacy) extemporaneously-compounded S8 product
To apply for authorisation to prescribe or supply an extemporaneously-compounded Schedule 8 product or an unregistered S8 medicine (other than a cannabis medicine) complete an

Cannabis medicines
To apply for authorisation to prescribe or supply a cannabis-based product for therapeutic use complete an

Esketamine or ketamine for treatment resistant depression
To apply for authorisation to prescribe esketamine, complete an

To apply for authorisation to prescribe and supply a substance for the purpose of human research complete an

Psilocybine or MDMA
To prescribe psilocybine for the treatment of depression resistant to other treatments, or MDMA for the treatment of post-traumatic stress disorder (PTSD), a prescriber with approval under the TGA Authorised Prescriber scheme will require an authority from NSW Ministry of Health.
