​From 1 August 2016, new application forms for authority to prescribe Schedule 8 (S8) drugs have been rolled out to replace the old 'Form 1'.  The new forms are designed to provide clarity on what is required for an application and contain specific questions depending on what drug and indication you are applying for.

Applications received on Form 1 may result in delays to the processing of the application.

Pain management

To apply for authorsation to prescribe a Schedule 8 opioid (e.g. oxycodone, morphine or fentanyl) or another S8 drug for the management of pain in a patient, complete the form​  Application for Authority to Prescribe a Schedule 8 Drug - Pain Management.

Attention Deficit Hyperactivity Disorder (ADHD) in a child or adolescent

To apply for authorisation to prescribe a psychostimulant (dexamfetamine, lisdexafetamine or methlyphenidate) for the management of ADHD in a child or adolescent patient, complete the form​  Application for Authority to Prescribe a Psychostimulant for Attention Deficit Hyperactivity Disorder (ADHD) in a Child or Adolescent.

To apply for authorisation to prescribe a high dose psychostimulant for the management of ADHD in a chlid or adolescent, complete the form  Application for Authority to Prescribe a Psychostimulant in a High Dose for the Treatment of Attention Deficit Hyperactivity Disorder (ADHD) in a Child or Adolescent.

Attention Deficit Hyperactivity Disorder (ADHD) in an adult

To apply for authorisation to prescribe a psychostimulant (dexamfetamine, lisdexafetamine or methylphenidate) for the management of ADHD in an adult patient, complete the form​  Application for Authority to Prescribe a Schedule 8 Drug - Psychostimulant.

Psychostimulant for treatment of a diagnosis other than ADHD

To apply for authorisation to prescribe a psychostimulant for the management of a diagnosis other than ADHD in an adult or a child patient, complete the form  Application for Authority to Prescribe a Schedule 8 Drug - PsychostimulantApplication for Authority to Prescribe a Schedule 8 Drug - Psychostimulant.
 

Alprazolam and flunitrazepam

To apply for authorisation to prescribe alprazolam or flunitrazepam, complete the form  Application for Authority to Prescribe a Schedule 8 Drug - Alprazolam or FlunitrazepamApplication for Authority to Prescribe a Schedule 8 Drug - Alprazolam or Flunitrazepam.

NSW Opioid Treatment Program (OTP)

Forms associated with the NSW Opioid Treatment Program (OTP) are now PDF interactive and can be completed electronically before printing and faxing to the Pharmaceutical Regulatory Unit.

From 1 August 2016, applications for authority to prescribe methadone or buprenorphine under the NSW OTP are to be made using one form (rather than a separate form for each drug). To apply for authorisation complete the form  Application for Authority to Prescribe Methadone or Buprenorphine under the NSW Opioid Treatment Program (OTP).

Interstate prescribers who wish to temporarily transfer a patient to the NSW OTP are required to complete the form Application for Temporary Interstate Transfer to NSW Opioid Treatment Program.

To exit a person from the NSW OTP complete the form  Exit from Methadone or Buprenorphine Treatment under the NSW Opioid Treatment Program.

Certain restricted substances

To apply for authorisation to prescribe the certain restricted substances (acitretin, clomiphene, cyclofenil, dinoprost, dinoprostone, etretinate, follitropin beta, isotretinoin for oral use, luteinising hormone, tretinoin for oral use, urofollitrophin (human follicle stimulating hormone)) complete the form  Application for Authority to Prescribe Certain Restricted SubstancesApplication for Authority to Prescribe Certain Restricted Substances.

Unregistered or (pharmacy) extemporaneously-compounded S8 product

From 1 August 2016 prior written authority from the Secretary of NSW Health is required to prescribe or compound any unregistered S8 medicine i.e. any product other than a TGA-registered medicine (i.e. those labelled with an AUST R number). To apply complete the form  Application for Authority to Prescribe an Unregistered or (Pharmacy) Extemporaneously-Compounded Schedule 8 Product.

Cannabis medicines 

From 1 August 2016 a medical practitioner may apply to NSW Health to prescribe or supply a cannabis-based product for therapeutic use. To make an application complete the form  Application for Authority to Prescribe and Supply a Cannabis-based Product.
