Social Impact Investment

The NSW Government is committed to delivering world-class healthcare to the people of NSW. From 2017 to 2024, NSW Health partnered with the Office of Social Impact investment to commission outcome-based programs to enhance community palliative care and mental health care: 

• Silver Chain Co​mmunity Palliative Care Service​​​: Silver Chain Group provided enhanced community-based palliative care services to people in Western Sydney from July 2017 to June 2024. This service has now transitioned to Western Sydney LHD. • Resolve Pro​gram​: Flourish Australia​ provided enhanced community-based care for vulnerable people with mental health conditions in Nepean Blue Mountains and Western NSW LHDs from September 2017 to September 2024. This program was led by peer workers and aimed to improve the well-being of individuals experiencing severe, persistent mental health issues by providing recovery focused care in the community. ​
