It is the responsibility of the current authorised prescriber to provide the clinical information required for safe transfer of care. As part of practitioner duty of care, prescribers should ensure all clinical information is available to the receiving team in a timely way. Clinics that are relocating or closing need to advise the Pharmaceutical Regulatory Unit in writing atMOH-OTP@health.nsw.gov.au.

Practitioners should discuss all treatment options and referral pathways available with clients (including Local Health District clinics and community prescribers) and assist with client transfer of care.
